Trinity 247 Care is seeking an organised and compassionate Care Coordinator to join our friendly team in Jersey. You will coordinate staff rotas and care visits, respond to changes and emergencies, maintain accurate records, communicate with clients, families and care staff, and help ensure safe, reliable, person-centred care. You will also support recruitment, staff onboarding, compliance checks, care-plan reviews and quality monitoring.
Applicants should have experience in health or social care, strong communication and organisational skills, confidence using digital systems, and the ability to manage competing priorities. Supervisory or care-coordination experience is desirable. A full driving licence and access to a car are required.
Applicants must already have the right to live and work in Jersey, as sponsorship is not available.
