Your set rota will involve supporting adults with mental health needs to live safely and independently in their own homes or supported living.
You will provide person-centred care, emotional support, medication assistance, daily living support, personal care and community access, while maintaining accurate records and promoting recovery, dignity, choice and independence.
Responsibilities include, but are not limited to, writing and reviewing care plans, risk assessments and other care documentation, and supporting the Team Leader with meetings, handovers and team communication.
A caring, reliable and compassionate approach is essential. QCF Level 2 or equivalent is advantageous. A valid driving licence is essential, with 1–2 years’ experience in a similar role required.
