Family Nursing & Home Care (FNHC) District Nurses provide expert care to patients over the age of 18 in individual homes, residences and clinic settings. District Nurses support islanders to help them avoid the onset and reduce the impact of illness. They work with people to try to limit disability or incapacity and shorten the duration of illnesses. They also support the management of long-term conditions and have extensive expertise in palliative and end of life care. The District Nurse Team Lead provides professional leadership and is accountable for the management of a team of Community Nurses and support staff in Adult Services You will be a Registered Nurse with the District Nursing Specialist Practice Qualification (DNSPQ)
